Various bottles of spray chemicals not properly labeled (corrected). Sanitizer for using the 3-bay sink dishwashing method is not properly hooked up to tubing for dispensing to sink. Corrected. Fryer baskets noted with grease and food debris accumulation. Interiors of reach in coolers at pizza prep area and back of the house sub area, not clean. Interiors of reach in freezers in kitchen noted with accumulation of food crumbs. Fan guards in walk in cooler not clean. Exterior of crock pot holding tomato sauce not clean, buildup of dried on food noted. Hand wash sink not draining. Men's and women's toilet rooms not cleaned in general. Floor beneath shelves in walk in cooler not clean. Floor and walls in main kitchen not cleaned in general. Walls in pizza prep area not cleaned in general. Several ceiling tiles throughout establishment stained and in poor repair.

Approximately 8 lb. pan of chili improperly cooled. Per interview with staff, Chili cooled in crockpot to room temperature and then placed in reach-in cooler in covered pan overnight. Chili voluntarily discarded (corrected). Three bags of potatoes stored directly on floor in dry goods pantry. Bags moved off floor, corrected during inspection. Three buckets of sliced potatoes stored directly on floor in walk-in cooler. Bottles of water and oil on grill not labeled. Bottles labeled during inspection (corrected). Scoops handles stored directly product in flour and sugar bins. Two containers of haddock fillets noted improperly thawing. Fillets measured at 32 - 45 degrees. Frozen fillets moved under running water and thawed fillets located on counter moved to reach-in cooler (corrected). Cardboard used to line shelving in food pantry. (not smooth and easily cleanable surface). Floor at doorway to walk-in cooler in poor repair and sealed with duct tape. Not smooth and easily cleanable. Three bay wash sink not set up properly. Sink was set up for washing, sanitizing, and rinsing. The process was corrected to washing, rinsing, and sanitizing. Staff were advised to follow the established procedure, corrected during inspection. Test strips for checking sanitizing solution not available during inspection. Wiping cloths not stored in sanitizing bucket, sanitizing bucket set up during inspection, corrected. Interior of upright Kenmore freezer in kitchen noted unclean with accumulated food debris. Fluorescent light fixture in kitchen not properly shielded. Air circulation fans in kitchen and by reach -in freezer noted with an accumulation of dust. Two hood filters missing from ventilation hood.
