Critical violations: Tomato slices at 45 degrees, shredded lettuce at 45 degrees and shredded cheese at 45 degrees in cold-holding unit; foods in cold holding should be at 41 degrees or below.
Noncritical violations: Low boy unit with back-up for make line has a buildup of ice on the right side vents and is not holding food at an internal temperature of 41 degrees or below. Soda dispensing machine has buildup of dried syrup around nozzles. Posted retail food permit has expired, but facility has a valid permit.

Critical violations: Open coffee cup at prep table. No handwashing plumbed in and not connected, using three-compartment sink. Tomatoes 45 degrees, guacamole 44 degrees, pico 44 degrees, cold hold unit. Turned down. All food in fridge needs to have a date mark/time stamp.
Noncritical violations: Onions and peppers were at 115 degrees, cheese at 65 degrees, meat at 135 degrees in hot holding; foods must be kept at 135 degrees or above in hot holding.

Critical violations: Employee handled cellphone and then put on gloves without washing hands.

Noncritical violations: Paint peeling from the walls in multiple places in the dish room. There is a buildup of ice on product boxes stored under the condensation catch pan of the commodities freezer.

Critical violations: Jalapeno pieces were in the bottom of front handwashing sink, repeat violation.

Critical violations: Dish machine was not dispensing sanitizer liquid during final rinse cycle. Laundry detergent stored under three-compartment sink.
Noncritical violations: Scoops for rice are being stored in room temperature water. Two tubs holding dry noodles and wontons are cracked on corner. Black growth on soda nozzles.
Qdoba Mexican Grill
2005 S. Promenade Blvd., Suite 100, Rogers
Critical violations: No soap available at a handwashing sink. Steak in hot well near grill was at 111 degrees and pinto beans were at 128 degrees.
Noncritical violations: Warmer handle needs to be replaced, door not latching or sealing properly and is allowing heat to escape. Retail food permit is not posted in facility, repeat violation.
