Restaurant Ratings Most Violations: January 14 to January 19 - WFLA

TAMPA, Fla. (WFLA) - The following Tampa Bay area restaurants received the most violations during inspections conducted January 14 to January 19.

Nickels Diner at 6403 Massachusetts Ave. in New Port Richey, 38 violations

  • The bathroom facility was not clean, this includes the toilets, urinals and hand wash sinks.
  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included shell eggs at 72°F.
  • Commercially processed ready-to-eat and potentially hazardous food was opened and held for more than 24 hours and was not properly date marked after opening. This included cottage cheese, milk and shredded cheddar/mozzarella cheese mix.

Oysters Restaurant at 606 NE US HWY 19 in Crystal River, 37 violations

  • Potentially hazardous cooked food was not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within a total of 6 hours. This included: collard greens 44°F, gravy 44°F, rice 44°F and beef 46°F.
  • Roach activity was present as evidenced by live roaches found. Five live roaches were found on the reach in cooler door gasket, 2 on the bottom ride of the steam table and one live roach was found on the side of the freezer on cook line.
  • Oyster tags were not marked with the last date served.

Tonbo Hibachi Grille LLC at 9310 US HWY 19 N in Port Richey, 27 violations

  • The wall was soiled with an accumulation of black debris in the dishwashing area.
  • Potentially hazardous food was left at room temperature to cool. This included fried fish, fried sushi roll and rice.
  • Cut carrots, peeled onions and crab cakes were stored and not covered in the walk in freezer.

Ban Thai & Sushi Inc. at 2519 McMullen Booth Rd. #506 in Clearwater, 27 violations

  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included garlic and oil 81°F and shell eggs.
  • Potentially hazardous covered food was stacked in deep plastic containers while cooling. This included cooked chicken.
  • A employee handled soiled utensils and then handled clean bowls without washing their hands.

Whaley’s Blazin BBQ at 911 Tomlinson Dr. in Lutz, 31 violations

  • Cooked meats were held hot at less than 135 degrees Fahrenheit or above. This included sausage pork 125°F and chicken 123°F.
  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included chicken cooked 89°F.
  • The cutting boards were soiled. The prep tables were soiled, as well as the carving knives and the smoker had a heavily soiled interior.

Chens Garden at 2665 N. Forest Ridge Blvd. in Hernando, 25 violations

  • Raw pork and chicken was stored over ready-to-eat cooked shrimp in the walk-in cooler. Raw beef was stored over shrimp toast in the vertical freezer.
  • A Stop Sale was issued due to food not being in a wholesome, sound condition. There were five 46 oz cans of pineapple juice that were dented on the seam.
  • Potentially hazardous food was left at room temperature to cool. This included cooked chicken 71°F and rice 57°F.

Mr. & Mrs. Crab at 7550 US HWY 19 N in Pinellas Park, 26 violations

  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: snow crab 46°F, king crab 46°F, blue crab 46°F and buttermilk 59°F.
  • Raw animal food was stored over ready-to-eat food. Raw beef was stored over cooked corn. Raw fish was store above unwashed potatoes.
  • A Stop Sale was issued due to food not being in a wholesome, sound condition. A piece of raw fish was observed in the same container as uncooked celery.

Ginger Indian Cuisine at 13031 Cortez Blvd. in Brooksville, 54 violations

  • Potentially hazardous cold foods in the walk-in cooler were held at greater than 41 degrees Fahrenheit. This included chicken 44°F, beef 44°F, rice 44°F, cut cabbage 44°F, tofu 44°F, sauce 44°F and curries 44°F.
  • A Stop Sale was issued due to food not being in a wholesome, sound condition. Chocolate milk was separated and spoiled.
  • An accumulation of black/green mold-like substance was found in the interior of the ice machine.
