American Golf Club, 100 Woodland Dr, Vero Beach: Had 21 violations including dead roaches on premises, accumulation of black/green mold-like substance in the interior of the ice machine, employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing hands, handwash sink used for purposes other than handwashing, encrusted material on can opener blade. Followup inspection passed with 5 violations including cutting board with cut marks and no longer cleanable, accumulation of black/green mold-like substance in the interior of the ice machine and old labels stuck to food containers after cleaning.
Siam Orchid Restaurant, 762 21 St, Vero Beach: Had 20 violations including cardboard used to line food-contact shelves, soiled floor in freezer storage, grease accumulated on floor and under equipment, unrefrigerated krab rangoon, dish machine not sanitizing properly, black/green mold on ice bin near sushi bar area. Followup inspection required.
Dario's Trattoria, 1555 Ocean Drive, Indian River Shores: Had 16 violations including can opener blade not kept sharp, case of food stored on the floor, food manager certification expired, no paper towels or mechanical hand drying device provided at handwash sink, no soap provided at handwash sink, slicer blade soiled with old food debris. Followup inspection required.
Bamboo Court, 532 21 St, Vero Beach: Had 16 violations including flour and rice stored uncovered, food stored on the floor, dented/rusted cans, food-contact surfaces not sanitized after cleaning, no proof of required state approved employee training provided for any employees. Followup inspection required.
Di'michelli's Catering Inc., 5300 73 St, Vero Beach: Had 16 violations including ready-to-eat, potentially hazardous (time/temperature control for safety) food prepared onsite and held more than 24 hours not properly date marked, no proof of required state approved employee training provided for employee hired more than 60 days ago, hot water not provided/shut off at employee handwash sink, cutting board(s) stained/soiled, no suitable facilities provided to store employee clothing and other possessions. Followup inspection required.
Country Ham N' Eggs, 478 US Hwy 1, Sebastian: Had 15 violations including encrusted food on the can opener, accumulation of black/green mold-like substance inside the ice bin, small flying insects, raw shelled eggs stored over ready-to-eat hot foods on the cook line, raw unwrapped ground beef and liver stored over potatoes in the reach in chest freezer, plumbing system in disrepair. Passed followup inspection.
Boston Market #1164, 1775 Us Hwy 1, Vero Beach: Failed an inspection with 15 violations on Feb. 13. Passed followup inspection on Feb. 21 with 4 violations including lights not working, soiled cutting boards and rusty walk-in cooler/freezer shelves.
Martin County
Mulligans Beach House, 131 Sw Flagler Ave, Stuart: Failed a Feb. 21 inspection with 17 violations. Met inspection standards on a followup with 5 violations including missing lights, leaky ceilings and a soiled soda gun.
Mondo Wood Fire Pizza, 950 SE Indian St, Stuart: Had 16 violations including employee with no hair restraint while engaging in food preparation, plumbing system in disrepair, potentially hazardous cold food held at more than 41 degrees Fahrenheit, raw calamari stored over ready-to-eat meats in cook line reach-in cooler, proof of required state approved employee training not available for some employees. Followup inspection required.
Dunkin Donuts/Baskin Robbins, 6001 SE Federal Hwy, Stuart: Had 15 violations, including no proof of required state approved employee training provided for any employees, interior of reach-in cooler soiled with food residue, handwashing sink used as a dump sink, potentially hazardous cold foods held at above 41 degrees Fahrenheit, ice buildup in walk-in freezer. Passed followup inspection.
St. Lucie County
Casa Azteca Mexican Restaurant, 101 N 2 St, Fort Pierce: Had 27 violations including can opener blade not kept sharp, food debris on exterior of microwave, interior of microwave soiled with encrusted food debris, employee failed to wash hands before preparing food, accumulation of black/green mold-like substance in ice bin, cutting boards stained/soiled. Followup inspection required.
China Star Restaurant, 5485 NW St James Drive, Port St Lucie: Had 26 violations including cardboard used to line food contact shelves, employee wearing jewelry while preparing food, uncovered flour stored by mop sink, uncovered food stored in walk-in cooler, bowl of raw chicken stored on top of bucket of onions, handwash sink blocked by jugs of soy sauce and oil, hot water turned off at handwash sink. Followup inspection required.
Ruffino's Italian Food and Pizzeria, 1145 SE Port St Lucie Blvd, Port Saint Lucie: Had 23 violations including ceiling tiles in disrepair, duct tape used to repair a nonfood contact surface, dust on food storage shelves, potentially hazardous cold foods held at more than 41 degrees Fahrenheit and encrusted material on can opener blade. Followup met inspection standards with 11 violations.
Club Med Main Dining & Infinity Pool Bar, 3500 SE Morningside Blvd, Port St Lucie: Had 21 violations including dented/rusted cans present, potentially hazardous cold food held at above 41 degrees Fahrenheit, potentially hazardous hot food held at below 135 degrees Fahrenheit, food stored on the floor, old labels stuck to food containers after cleaning, meat and lemon slicer blade soiled with old food debris.
Ninja Thai Asian Fusion Restaurant, 899 E Prima Vista Blvd Ste Ab, Port St. Lucie: Had 20 violations including food stored on the floor in dry storage area, exterior door has a gap at the threshold that opens to the outside, old labels stuck to food containers after cleaning, dented/rusted cans present, cutting boards stained/soiled. Passed followup inspection with 3 violations.
Hopcat, 10553 SW Meeting St, Port St Lucie: Had 18 violations including a reach-in freezer soiled with food, a handwash sink not accessible for employee use due to being blocked by soiled dishes and grease holder, encrusted material on can opener blade, raw chicken, raw hamburger and raw beef held at above 41 degrees Fahrenheit, raw scallions not washed before preparation and floor areas covered with standing water. Followup inspection needed.
Main Bar, 3500 Morningside Blvd, Port St Lucie: Had 17 violations including old food stuck to clean dishware/utensils, accumulation of food debris on handwashing sink, employees using handwash sink as dump sink, no paper towels provided at handwash sink. Followup inspection required.
