Lemons noted with mold. Butter with mold. Broth with vegetables with mold. All from walk-in cooler, all discarded. Numerous potentially hazardous foods stored in walk in cooler at temps between 50 and 57 degrees for an undetermined amount of time. All discarded voluntarily upon request. 48 dozen raw whole shell eggs, 40 pounds of raw chicken, 4 quarts of half and half creamer, 40 lbs. of tofu, and a large container of peanut paste. Ambient air temperature is between 55- and 60-degrees during inspection. All food in question temped between 50 and 57 degrees. Walk in cooler not maintained properly to keep potentially hazardous foods at 45 degrees or less. Walls, wire shelving, and plastic milk crates in walk in cooler not clean, mold noted. Sides of cooking equipment at the cookline not clean. Grease buildup noted. Hand wash sink not draining. Hand wash sink lacks soap and paper towels. Also, sink is blocked by chair and tray rack, not easily accessible. Floors underneath equipment at the cookline not clean. Excessive grease accumulation and food debris noted. Hoods and vents over the cookline not clean. Large amount of non-functional equipment stored in the kitchen has caused the kitchen to become cluttered.

Soft cheeses noted between 50-54 degrees in top left portion of preparation cooler - put on ice. Containers of mashed potatoes noted at 120 degrees covered and cooling in walk-in cooler - uncovered. Top left portion of reach-in preparation cooler not operating properly to maintain temperature at or below 45 degrees. Three bay sink at bar not set up for wash, rinse, sanitize - wash was in middle bay - corrected. Exterior of some equipment not clean.

Raw, whole shell eggs (partial carton) noted stored next to sliced deli meats and other ready-to-eat foods (corrected - moved to lower shelf next to raw meat). Male food workers (2) lacked hair restraints at time of inspection (corrected). Hand sink basin noted obstructed at time of inspection with multiple items stored in hand sink (metal pan, worn pair of sanitary gloves, and a paper towel) (corrected - all items removed from hand sink basin to promote accessibility and frequent hand washing). Hand soap dispenser noted not functional and hand sink lacked any other hand soap at time of inspection (corrected - container of dish soap placed at hand sink for a temporary solution; hand soap dispenser must be repaired or replaced). Garbage and cardboard noted placed on ground surface outside rear door. Flies noted present in kitchen (immediate service required by a licensed pesticide applicator). Floor drains (2) noted not clean. Kitchen floors noted greasy and not clean at time of inspection.

Cream base soup noted at 120 degrees in hot holding unit and vegetable-based soup noted at 95 degrees in hot holding unit, not properly reheated to 165 or above on stove before putting in holding units (corrected). Raw fish noted thawing at room temperature at end of cook line - corrected. Interior and exterior of reach-in cooler below meat slicer not clean.
