Wrapped tofu on a stick and whole lemons stored next to and below packaged raw meats (corrected-tofu and lemons moved). Edible fungus (Wood Fungus) and partially peeled Sweet Potato noted in prep top cooler at 56 degrees and 53 degrees for less than two hours according to cook (corrected - product put into walk in cooler). Food pans in prep top cooler are stacked without hard covers between them-food not protected. Bulk cake flour, starch, and sugar bags left open following use-food not protected. Poly cutting board at prep top and smaller poly board both noted with heavy stains/cut marks-in poor repair. Meat slicer blade guard and blade not clean with residual food soil. Magnetic knife and utensil holder not kept clean. Used, unclean utensils stored with clean utensils. Bottom of cookline low cooler noted with standing water. Cookline low cooler door gaskets not clean. All lower shelving and cart at cookline not clean. Floor areas along baseboards, under equipment, racks and behind chest freezers not clean with environmental and food soil debris. Some wall areas not clean.
