Violations:
- One gallon of whole milk that was being used to serve consumers by the glass had an expired "sell by date". Discarded.
- Wooden frame that supports a countertop, has paint peeling and is no longer non-absorbent and easily cleanable.
- The chicken walk-in cooler condensing unit fan covers have a black moist residue accumulation.
- Wall mounted squirrel cage-type fan unit, located above the clean dish table, has dust and black moist residue with the potential to contaminate clean utensils and dishes.
- The Person-in-Charge is not performing the duties as required by the Pa. Food Code to actively manage food safety in the facility.
- Food employees observed in food prep areas wearing bracelets and watches.
- Food employees observed in food prep not wearing beard covers.
- Observed a whitish mildew-type residue on the plastic scrapper blade of the food chopper. Cleaned.
- Observed dried food residue accumulation on the food slicer. Cleaned.
- The upstairs and downstairs ice machine deflector plates had a dark moist residue accumulation. Cleaned. Repeat violation.
- Observed single-service, single-use articles stored in downstairs storage area, directly on the floor, and not six inches above the floor.

- Litter and debris under and around equipment.
- Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was zero ppm, and not 50-100 ppm as required.
