AL.com has gone through every restaurant health inspection in June to bring you the 10 establishments with the worst ratings.
Our starting point is an 85 score -- the threshold that prompts a reinspection within 60 days.
Alongside a restaurant’s score is how many critical violations they received. These are marked in red on the county health department’s website and are either worth five or four point deductions.
These most serious infractions can range from food safety training failures to roach and rodent droppings.
Here are the 10 Jefferson County restaurants with the worst health inspection scores for June. Scores are arranged from highest to lowest:
Black Sheep Kitchen, 81 Church St., Mountain Brook
Score: 85
Critical violations: 3
Black Sheep Kitchen had 15 points knocked off its rating after a June 3 inspection.
The eatery had two five-point deductions, including for having pans of chicken breasts that were cooled 60 degrees lower than acceptable levels and not labeling a toxic solution that was not in its original container.
The person in charge at the time of the inspection also couldn’t locate chlorine test strips for the sanitizer, which cost Black Sheep Kitchen four points.
Subway (inside Walmart), 1600 Montclair Rd., Birmingham
Score: 85
Critical violations: 3
The Subway lost 13 points off its score from three critical violations after an inspector visited the establishment June 24.
Subway was docked five points for storing food above acceptable temperatures.
It also had two four-point violations. One was for only providing food safety training to two out of six employees on the weekly schedule. The other was for having water turned off due to a detached pipe.
A&A Quick Stop, 4356 1st Ave. North, Birmingham
Score: 84
Critical violations: 3
This convenience store was nailed with three critical violations worth 14 points following a June 24 inspection.
Two of those were of the five-point variety – the most serious violations that the health department cites. Both were for not storing food at proper temperatures.
A&A Quick Stop was also cited for not having a food thermometer to check temperatures.
Sol Azteca, 1360 Montgomery Highway, Vestavia Hills
Score: 82
Critical violations: 2 Sol Azteca was cited for two critical violations that deducted nine points off its score after a June 24 inspection.
Its most serious infraction was for not being able to verify that a new employee was told what the establishment’s policy was for having symptoms of diseases that could be transferred through food handling. The infraction cost Sol Azteca five points.
The Mexican restaurant was docked another four points for not providing “vomit and diarrhea clean-up procedures” to employees.
Basil, 1101 Dunston Ave., Birmingham
Score: 80
Critical violations: 3
Basil had 13 points slashed off its score because of critical violations after a June 12 visit from a health inspector.
Its most serious violation was for storing meat, cheese, house-made ranch and eggs between five and nine degrees above acceptable temperatures, which cost Basil five points.
Basil was docked another four points for having a soiled ice machine and a manager who couldn’t verify that they were certified in food safety.
Piggly Wiggly (deli section), 3314 Clairmont Ave., Birmingham
Score:79
Critical violations: 4
The deli section of the Piggly Wiggly in Highland Park had 17 points cut from its score from four critical violations after a June 6 inspection.
The Piggly Wiggly had only one five-point critical violation for storing food in the walk-in cooler between six and seven degrees above acceptable levels. The inspector told the person in charge not to use the cooler until it was repaired.
Meanwhile, several foods in the cooler were not labeled, which cost the grocery store another four points off its score.
The Piggly Wiggly had two other four-point violations. It was cited for not having running water in a handsink and not providing food safety training for all employees.
Momma Goldberg’s, 2829 18th St. South, Homewood
Score: 77
Critical violations: 4
The sandwich shop in downtown Homewood racked up four critical violations that cost it 17 points off its health score after a June 20 inspection.
Momma Goldberg’s was docked five points for not labeling a toxic item in a spray bottle that turned out to be sanitizer.
The inspector also noticed an “accumulation of grime and residue” on the meat slicer and beverage nozzles – a four-point deduction.
A hand sink also did not have adequate water pressure and was not at a temperature of at least 100 degrees, the inspector noted.
Momma Goldberg’s was also cited for not providing food safety training for employees that had been with the shop for at least three weeks.
Sonic Drive-in of Fairfield #4794, 6625 Aaron Aranov Dr., Fairfield
Score: 74
Critical violations: 5
The Fairfield Sonic lost 24 points off its score because of critical violations after a June 12 inspection, including four five-point infractions.
One of those violations was for a “large amount of grease spills on the ground” by the dumpster.
The inspector noted that an employee did not change gloves when they handled a hamburger bun after picking a wrapper off the floor.
Hot dogs were also found to be stored 20 degrees below acceptable temperatures and the inspector couldn’t verify that employees were trained on food safety.
Circle K #2723818, 1685 Montclair Rd., Birmingham
Score: 73
Critical violations: 5
This Circle K had 21 points knocked off its score after a visit from a health inspector on June 24.
The convenience store was docked four points after the inspector discovered rodent droppings underneath the slushie machine “with a foul odor.”
The inspector also found that the drink nozzles were “heavily soiled,” which cost Circle K another four points.
The women’s restroom was missing soap, which tacked on another four-point deduction.
Five points were taken off the convenience store’s score for not providing a copy of the employee health policy.
Sonic Drive-in, 7701 Parkway Drive, Leeds
Score: 71
Critical violations: 4
The second Sonic on this month’s list had the lowest health inspection score of any eatery in Jefferson County.
All four of the Leeds Sonic’s critical violations were for the maximum five points, cutting its score by 20 just from critical infractions alone after a June 20 inspection.
Dairy dessert mix was being stored at 53 degrees – 12 degrees above acceptable levels, the inspector noted. The soft serve machine was closed by the manager during the inspection and the mix was thrown out.
The inspector also found that the dessert freezer wasn’t being washed at least every day.
Sonic could not verify that a new employee on duty during the inspection was given food safety training.
The establishment’s last five-point deduction came from not labeling a toxic chemical that was transferred out of its original container.
