Violations:

--Powdered sugar ingredient storage containers, above the prep unit and in the shelving area, is not labeled with the common name of the food.

--Scoop being stored in the ice machine with handle touching and/or buried beneath the ice.

--Observed boxes of food stored directly on the floor in the walk-in freezer, rather than six inches off of the floor as required.

--Food held in prep unit was held at >58F, rather than 41F or below as required.

--Observed tomatoes stored on top of paper towels within the prep unit.

--Lettuce observed in the two-door refrigerator, slimy to the touch.

--Observed pretzel salt stored uncovered

--Observed excessive ice build up in both ice cream freezers

--Observed prep unit holding at >58F

--Observed unapproved mouse trap being used within dry storage area of facility
